Why Bio–Identical Hormones?

As more and more studies, reports and articles describe the many health problems that can be caused by Hormone Replacement Therapy (HRT), more and more women are refusing to take these synthetic products that are made by pharmaceutical companies.

So what options do women of all ages have if they don’t want to suffer with severe symptoms and health problems that are caused by an imbalance and/or a decrease in their sex hormones?

As a Naturopathic Doctor, I have used, and still use glandular remedies (ex bovine ovaries, bovine uterus), herbal remedies and/or homeopathic remedies to treat the symptoms and hormonal imbalances in women. During the reproductive years, natural remedies can stimulate the woman’s body (ex. ovaries) to make more hormones and help relieve symptoms.

During perimenopause, menopause and postmenopause, natural remedies can sometimes help relieve a woman’s symptoms, but can never replace the hormones at the levels that a woman’s body made during the reproductive years. Only Biologically Identical (Bio–Identical) Hormones can do this.

It has also been found that, taking herbal remedies, without knowing what the actual levels of a woman’s hormones are, can lead to even more of an imbalance in a woman’s hormones. This is a significant concern, as women have been shown to have an increased risk of breast, endometrial and/or uterine cancer if they do not have a proper progesterone to estrogen ratio.

When Dr. John Lee’s book, What Your Doctor May Not Tell You about Menopause introduced women to the idea of using Biologically Identical (Bio–Identical) Hormones, many women decided that they wanted to use this form of hormones. According to Dr. John Lee, Progesterone was the mother hormone in a woman’s body. He stated that all a woman with hormonal decreases or hormonal imbalance had to do was take Biologically Identical (Bio–Identical) Progesterone, and the body would convert it into any and all the other sex hormones giving the woman’s body the proper balance of sex hormones.

Many women who obtained the Biologically Identical (Bio–Identical) Progesterone were amazed at how good / healthy they felt on this hormone, and even were quoted in newspaper articles as saying that they would do anything to obtain it (ex. buy it across the border, buy it under the counter from health food stores), if the British Columbia Government banned its use.

Now, Biologically Identical (Bio–Identical) Hormones can be legally obtained in British Columbia with a prescription from a woman’s medical doctor (unfortunately, a naturopath cannot legally prescribe them in British Columbia). Unfortunately, many medical doctors are still unwilling to prescribe them.

History and Approach

The question about the safety of using Biologically Identical Hormones keeps coming up, even though they were first used over 5,000 years ago by Chinese Emperors and their wives and concubines. At that time, however, the Biologically Identical (Bio–Identical) Hormones were prepared from the urine of adolescent boys and girls.

Biologically Identical (Bio–Identical) Hormones are now made from the Mexican Wild Yam and have been prescribed by progressive medical doctors for many years. For example, Dr. Jonathan V. Wright, MD has been prescribing them since the early 1980’s.

Lab testing is an extremely important step in determining whether a woman has an imbalance or balance in her sex hormones, what amounts she may need of one or more of the hormones (estrogen, progesterone or testosterone) and whether she has been prescribed the correct amounts of Biologically Identical (Bio–Identical) Hormones.

The standard blood tests done in laboratories only measures one sample in spite of the fact that a woman’s estrogens change every 6 to 18 hours, only measures Estradiol (which is only 10 to 20% of a woman’s estrogen — a woman’s body makes over 30 Estrogens with Estrone making up 10 to 20% and Estriol making up 60 to 80% of a woman’s estrogens). Also, the blood test does not show what the tissue levels of the sex hormones are — it only shows what free amounts are in the blood (much of the sex hormones are bound up in the blood).

Saliva testing has been around for many years and has proven reliability and reproducible results. I have been using them to test sex hormones since 1998. Before then, I relied upon the results of blood hormone tests and the symptoms that the patient was having. Then in 1998, I found out the importance of always using the saliva test to check the hormonal levels.

A Client's Story

A 51 year old woman came to see me specifically because she wanted to do the saliva test to check the level of her hormones. Her medical doctor could not order a blood test because she had slipped into menopause with absolutely no symptoms. She decided to do the saliva test as she had a family history of breast cancer and wanted to prevent herself from getting breast cancer, not just do monthly breast self examination and yearly mammograms and treat the cancer if it occurred.

The salivary lab test showed that she had excessive estrogen and relatively low progesterone even though she had no symptoms. Also, her progesterone to estrogen ratio (a mathematical calculation established by medical doctors) showed that this woman, who was the envy of all her female friends because she had slipped into menopause without any symptoms, was at risk for breast, endometrial and/or ovarian cancer. Therefore, I always get women to do a salivary hormone test and never trust just the woman's symptom picture.

Things To Be Aware Of

As I have done saliva hormone testing on many women who have followed Dr. John Lee's advice (and often their health practitioner's advice) and taken only biologically identical progesterone, I have found that the progesterone does not convert into the proper amounts of all the other hormones. Each woman is an individual, whose body makes, uses and breaks down her hormones at a rate which is different from other women. Therefore, ideally, I encourage every woman to get a baseline salivary hormone test done during their peak reproductive years in order for us to know what is their normal level of hormones. Otherwise, when I test and recommend dosages for their medical doctor to prescribe for them, I recommend the lowest dose of each of the hormones and help to establish the correct level of Biologically Identical (Bio–Identical) Hormones for the individual by saliva rechecks.
